Neglected cavities, fractures, gum disease like periodontitis, and one or more teeth that can weaken and fall. The most frequent solution today is the dental implant: according to the latest data from the Academy of Osseointegration, every year in Costa Rica several hundreds of thousands of implant surgeries are made, a fact that places our country at the level of the biggest countries to draw on the method.


Yet, around this technique so widespread (it is estimated that about two dentists out of three usually do it) still circulate fears and perplexities, fortunately almost always unjustified.

To dispel any doubt, we asked prominent Prosthodontist Dr. Alejandro Freer R., DDS; MDS; Prosthodontist of Freer Dental Implant Center to answer all the most widespread and crucial questions on the subject.

1. Can you place dental implants with infection or gum problems?

"In the case of infection, the implant can only be assembled after eliminating all traces of infection. If, on the other hand, one or more teeth are lost due to periodontitis, the most serious consequence of gingival diseases, once the implant has been inserted, the plaque must be scrupulously removed with toothpaste and a soft brush and using dental floss twice a day".

2. And if you suffer from osteoporosis?

"It can be done if you are not following bisphosphonate-based therapy. These drugs, in fact, have the side effect of making the jaw bone much more fragile and sensitive: by piercing it for the implant, therefore, there is a high risk of forming necrosis, ie that the bone dies, infecting the fabrics all around. Better always to warn the dentist first: he will evaluate what to do ".

3. Can smoking damage a dental implant?

"Nicotine causes a problem of blood supply in the tissues of the area where it intervenes, complicating the work of the dentist and shortening the duration of the implant over time. Those who smoke should at least reduce cigarettes drastically (no more than 5-6 per day). No even to electronic ones if they contain nicotine, albeit in small quantities ».

4. Can you insert if the bones of the mouth are thin?

«If the thickness of the mandibular bone is not sufficient, is that of the inferior arch, it is better to give up: the techniques to increase it through grafts are still in the initial phase. For the maxillary bone, that of the upper arch there are procedures consolidated for years to increase the thickness with the re-growth of the patient's bone ».

5. Are there any diseases that discourage?

"There are no pathologies that prevent the insertion of implants, first of all, the health of the mouth. The dentist should always be advised if you have heart disease or other chronic diseases: advanced diabetes, for example, promotes the onset of inflammation in the mouth ".

6. Will it be painful?

«The operation is painless thanks to the local anesthesia, which is that of simple obturation. No problem even after surgery: there are no swelling and scars. If you feel discomfort and for prophylaxis, the doctor will prescribe painkillers and antibiotics ».

7. Is there a period in which one remains without a tooth?

"If it is the case of an anterior tooth, after implantation a temporary crown is applied. For a molar (posterior) in the majority of the case, the implants it's left healing under the gums without a temporary, after about three to four months, it's placed the definitive one ".

8. Is there a risk of allergies or rejection?

"No, the material that is used (titanium) is totally non-allergenic and there is no risk that the body can" reject "it».

9. Can an infection come?

«Intervention for an implant may cause an operative bacterial infection (if done in conditions that are not completely sterile) or post-operative (if the patient has not taken correctly the antibiotics nor respected the rules of oral hygiene or if the implant was not placed as needs to be). Under normal conditions, when the intervention is carried out properly and the subsequent maintenance is respected to the end, infections occur in one case every 2,300. And if identified immediately, they can easily be cured ".

10. How often should checks be carried out?

«Depending on what he will consider appropriate, every 3 months and, later, if everything proceeds regularly, at intervals of 6 months ".

11. Will we talk and chew as before?

"If the surgery is performed correctly, the implant has no negative phonetic or masticatory consequences".

12. How long is the lifespan of an implant?

"According to statistics, the average life of a plant is 15 years, but in reality, the vast majority reaches 20 years and can even reach 25: everything depends on maintenance, that is, how constant the patient is in the daily cleaning".

13. What will the new teeth look like?

"From an aesthetic point of view, they will not be exactly identical to natural ones because they are ceramic. You can approach the same color shade, everything depends on the expertise of the dental technician that produces your case. 14. Is it a burdensome intervention?

"The implant post for a single tooth, working with a Surgeon Specialist who will be using only premium brands and their original abutment parts and supplies, cannot cost less than $800 (without crown).
