Since 2009 we at the Freer Dental Clinic have been considered a reference point for the Health of the Mouth and Smile of all our Patients. Among them, there are people who, due to total or partial loss of teeth, feel discomfort, embarrassment, even social discomfort in wearing a mobile denture.

Our task is simply to help them, but what alternatives to dentures?

In 2003 Dr. Paulo Malo, founder of the Malo Clinic in Lisbon, invented a new and customized solution aimed at replacing the mobile prosthesis: All on Four / All on Six.

Years passed and the All on Four / All on Six were the subject of studies by the dental community that validated them scientifically with numerous publications;
dental interventions conducted with success and full satisfaction on the part of patients around the world.

What problems do dentures cause?

The dentures are to be considered unstable mobile prostheses, attached to the gums by means of precarious adhesives, the discomforts are various as:

  • Insufficient stability of the teeth inside the mouth;
  • The annoyance of always having to remove the prosthesis for oral hygiene and for nocturnal sleep;
  • The denture is also considered painful;
  • Functional problems: it makes chewing difficult;
  • Aesthetic problems: it makes it difficult even to smile;
  • It can also induce bone reduction if carried out for several months or years;

What is Implantology All on Four and All on Six?

All on Four = All Teeth on Four Implants.

All on Six = All Teeth on Six Implants.

This is an Immediate Load Implantology intervention that allows placing within the same operation 4 or 6 Implants in strategic areas in the upper or lower arch. These implants will act as a support for a firm and resistant first temporary, then definitive prosthesis.

By Immediate Load, we mean an operation in which the teeth are extracted, after which the implants and temporary crowns will be installed after 24 hours. Immediate Loading is a fixed temporary acrylic bridge from the beginning of the treatment. 4 to 6 months later the definitively fixed bridge will be inserted.

All on Four and All on Six dental implants are made of titanium (titanium is a biocompatible material that does not cause foreign body reactions) that are inserted into the bone.

The implant will become part of the body itself and the arch will be completely rehabilitated.

Who is the treatment recommended for? 


In which cases do you recommend the All on Four and All on Six?


1 - Patients who wish to replace their dentures with fixed teeth.


2 - Patients with upper or lower teeth without teeth, or who have totally or partially lost all their teeth (total edentulism).

3 - Patients with periodontal pathology, damaged dental arches, and require total prosthetic extraction and rehabilitation.

4 - Patients for whom traditional implantology is not possible due to bone deficiency, or when implants cannot be inserted without a bone regeneration procedure in delicate areas such as the upper maxillary sinuses and the lower mandibular nerve.

5 - Patients with severe or very severe atrophy or demolition of the maxillary bone structures.

6 - Patients who have damaged teeth with the expectation of having to wear dentures sooner or later: exhausted dentition condition.

There are no contraindications to the patient's age, however, the intervention will be Optimal in Patients who have Good gingival support conditions, Dental stability, and masticatory occlusal balance, which do not have a bruxism condition and do not present serious forms of occlusion.

Between All on Four and All on Six, what is the best solution?

Your Dentist will tell you this after evaluating your specific case.

For the upper arch:

You can apply the All on Four / All on Six technique if this arch is damaged or compromised.


For the lower arch:

You can apply the All on Four / All on Six technique if this arch is damaged or compromised. This technique requires that the 4/6 implants be placed without the need for bone regeneration.


The prosthesis can be:
  1. Fixed with gingiva: this solution is adopted when the patient has bone resorption. The fake gum serves to prevent the fixed teeth from being too long. The gingiva of the prosthesis will be perfectly camouflaged with the real gum and to enjoy it will be the Smile of the Patient resulting in Harmonious and Natural.
  2. Fixed without gingiva: this solution is adopted when the bone is not reabsorbed or if the patient does not like fake gum.
How should I prepare for the All on Four / All on Six?

The intervention does not require specific preparation standards.

How is the treatment performed?

First session: aesthetic and functional diagnosis of the dentition. They are analyzed:
  1. tooth shape;
  2. tooth position;
  3. line of the Smile;
  4. soft tissue support;
  5. color;
  6. occlusion;
  7. chewing.
Preoperative study with examinations such as TAC Cone Beam 3D of the jaw and/or maxilla and Orthopanoramics (OPT orthopantomography). These are low-dose X-ray radiographic examinations carried out to assess the quantity and quality of bone tissue.

The patient's general state of health is evaluated.

The footprints of the dental arches are detected.

The Dentist will be very attentive to particular areas such as the mandibular nerve and the maxillary sinus that must not be touched by the operation.

Impressions of the dental arches are acquired: a fundamental phase to accurately determine the skeleton of the mandibles and jaws and simulate the implant, choosing in advance their inclination, width, and size.

The second session (Surgery): Periodontal Hygiene. To guarantee the success of the intervention, it is necessary to eliminate the bacterial plaque and tartar, both below and above the gum.

Extractions of any remaining compromised teeth, insertion of dental implants that will support the fixed prosthesis. In this phase, the gingival part is detached to allow total hygiene and correct integration of the structures.

Four or six implants (two or three pairs) are fixed on the right and left arches, along the perimeter of the gum in strategic areas on a definitive prosthesis that faithfully reproduces the dimensions and dimensions of the original dental arch.

The entire session is performed under IV Sedation.

Third Section: The All on Four / All on Six technique allows you to place an immediate load prosthesis with a minimum of 10 teeth. The prosthesis is designed so that the transition between the fake and natural gums is not visible. The holes through which the prosthetic screws pass (screws that anchor the prostheses to the implants) are closed with a composite of the same color as the teeth.

The temporary fixed dental prosthesis, with the Instant Load Implantology technique, is applied the day after, screwing it to the dental implants Thanks to very small passing lives. The provisional prosthesis, composed of 10 elements, will accompany the patient for the entire healing period (generally the duration is 3 months).

The implant anchor is guaranteed by 4 or at most 6 implants.

Even if the entire prosthesis is unique, it is possible to create the effect of interdental spaces by giving a natural and harmonic smile.

In a few days, there will be a full recovery from traumas, albeit minimal, linked to the inclusion of the 4/6 implants that occurred during the day.

After 3 months, the bone integration will be completed and the Patient Rehabilitation will take place with a Definitive Dental Prosthesis composed of a hybrid bridge made to measure 12 ceramic or titanium teeth with various types of aesthetic coatings and the integration of 'bone.

Will I feel pain during the surgery?

No, the operation is minimally invasive and is performed under IV Sedation, you will be sedated for all the surgery..

The All on Four Solution has been specifically designed to reduce pain as much as possible.

Even the swelling is reduced to a minimum.

How will I feel at the end of the intervention?

We advise you to start chewing 3 hours after surgery.

From the next day, you will be able to resume your normal activities, including those of relationship, experiencing, and immediately ascertaining the benefits of the fixed prosthesis!

Although most patients experience an immediate recovery, we recommend taking 4 to 5 days off from surgery before returning to your normal activities.

The soft tissue takes about a week to heal.

The osseointegration takes 3 to 4 months instead of for the bone to grow and to be welded around the implant

What other advice can we give you at the end of the intervention?

Do not practice sports until osseointegration.

Feeding: Prefer soft and cooled foods before putting them in your mouth.

Sleep: before going to sleep, make sure you lift the pillow, this will avoid the sensation of pulsation in the dental area.

Once entered, how long is the All on Four / All on Six systems?

If you keep regular checks by the dentist and correct oral hygiene at home, the implant may have no time limits!

The implants remain inside the organism permanently.

What can the dentist do for you?

Dr. Alejandro Freer will advise you and will instruct you both before and after the operation to ensure that the procedure is successful.

If you wish, for your greater comfort and tranquility, the dentist will explain the various phases of the operation that you will perform inside your mouth.

At the end of the intervention, you will release ice to keep on the operated area until the effect ceases.

What must be avoided is the danger of a peri-implantitis, or bacterial inflammation of the bone and gum around the implant: our dentist will give you advice on how to maintain good and correct oral hygiene at home

Does the service have a periodicity?

Yes, it is necessary to guarantee and maintain the hold of dental implants over time. Therefore it is necessary to carry out periodic checks every 3, 6, or 12 months depending on the case.

Besides excellent daily maintenance performed at home, periodic checks with regular hygiene sessions at least every 6 months will also be necessary.

If you feel strong pain in the days following the operation, swelling that shows no sign of abating, prolonged bleeding, don't hesitate to call us.

Who is this intervention not recommended for?
  1. Patients who have edentulism with bone insufficiency in the maxillary and mandibular level.
  2. Patients who do not have primary bone stability or soft bone tissue.
  3. Patients suffering from immune system disorders.
  4. Patients who carry out radiotherapy or Cure with bisphosphonates.
For this operation, they are not contraindicated, but smokers and bruxists (subjects who grind their teeth) are considered at risk.
